The following conditions would be challenging for both patients and their families: An older adult faces frequent outpatient visits, taking many drugs, rapid decline in physical or cognitive function, repeated emergency department visits or hospitalizations, or end-of-life stage with palliative care needs. Therefore, we look forward to advising on both health care and personal care, choosing the right care at the right time after discussing the goals of care with the patient and families. We also hope to help the patients to have a good transition in care between home and medical institutions.
